The Association pursues the following objectives: Improving the management and coordination of patient care and caregivers, avoiding unnecessary/ redundant examinations and consultations, and limiting inappropriate hospitalizations; in respect of the care plan established with the patient and all health professionals; Encouraging and promoting interprofessional and multidisciplinary work for patient care; Contributing to the development of the health-social system by strengthening the response of health professionals to the needs of patients; Strengthening and developing health promotion and prevention actions within the Commune of Meinier and the region; in line with the concept and action plans of the canton of Geneva; Providing the entire population of Meinier and surrounding areas, particularly vulnerable populations, with access to quality and local care; Providing interprofessional and coordinated primary care; Offering community health services and activities; for individuals, groups of individuals or more generally the inhabitants of the affected municipalities; Promoting patient home care; Promoting the training and interprofessionalism of the various health actors; Promoting an integrative, efficient and innovative health system; Participating in studies or research projects; Promoting collaborations with the various actors in the health-social field of the region.
