supports the fight against chronic diseases, effects of aging, disability situations and physical deconditioning, as well as their consequences for the individuals concerned. It promotes the mobility of these individuals in their daily lives. It respects the recognized principles of medical science, evidence-based practice and bio-psycho-social work. Its objectives are to: promote the mobility of individuals with chronic diseases and/or elderly and/or disabled and/or physically deconditioned; facilitate, promote and improve access to adapted physical activity and reduce the costs of individuals with chronic diseases and/or elderly and/or disabled and/or physically deconditioned; improve the quality and promote interprofessional care (assessments, care, adapted physical activities, clinical referrals, communication, therapeutic education, etc.) of individuals with chronic diseases and/or elderly and/or disabled and/or physically deconditioned; coordinate prevention and health promotion actions around chronic diseases and/or geriatrics and/or disability situations and/or physical deconditioning; train and inform health professionals around chronic diseases and/or geriatrics and/or disability situations and/or physical deconditioning; represent the interests of individuals with chronic diseases and/or elderly and/or disabled and/or physically deconditioned; collaborate with federal, regional, cantonal and local institutions with similar objectives. To achieve these objectives, the association may develop conventions, establish regulations and conclude contracts for its members or project beneficiaries.
