Association PRISM (Projet de Réseau Intégré de Soins aux Malades)

To support and provide individuals or institutions active in the field of health in the canton of Geneva with the necessary assistance and financial resources to develop care networks, ensuring continuity of care for patients with chronic and/or complex conditions, coordinating and improving care among all partners, supporting patients in their autonomy by networking local resources, and thus promoting a strengthening of the patient-physician relationship and the emergence of new forms of cooperation and new healthcare professions.
